According to Georgia State Patrol reports, on September 16, at approximately 11:35 p.m., a 2004 Dodge Ram1500 pickup truck and a 2004 Jeep Grand Cherokee were traveling east in the left lane of Highway 411. The driver of the Dodge stated the Jeep was slowing down and he was not able to stop the truck. The pickup struck the rear of the Jeep with its front. Both vehicles exited the north shoulder of the highway and came to an uncontrolled rest in the median facing north.

Driver of Dodge – Scottie Worfford, age 51 from Rockmart. He was injured and transported to Cartersville Medical Center. He was charged with three counts of Homicide by Vehicle (1st Degree), Serious Injury by Vehicle, Driving Under the Influence, and Following Too Closely.

Driver of Jeep – Deneisha Washington, age 25 from Dalton. She was fatally injured.
Back Seat Passenger – Octavian Bynum, age 11 months from Dalton. He was fatally injured.
Back Seat Passenger – Iyania Bynum, age 2 from Dalton. She was fatally injured.
Back Seat Passenger – Shamya Washington, age 8 from Dalton. She was airlifted to Egleston Hospital.
